It has been reported that upwards of 3% of patients on fluoroquinones,
e.g., ciprofloxacin, grepafloxacin, etc., report Agitation, insomnia,
and restlessness as adverse events with these drugs. Does anyone know
what the mechanism is behind these AEs?

Cipro has CNS effects of its own, but one thing that is often overlooked
(but well established) is the inhibition of caffeine metabolism:

CNS stimulation by fluoroquinolones has been attributed to GABA
antagonism.
