What is the appropriate PK/PD monitoring for Vancomycin, AUC orBack to the Top
time-above-MIC. I would believe the later would be more important since
Vanco has a short post-antibiotic effect (1,5hrs) and increasing the
concentration above the MIC does not markedlly increase the
bactericidal effect.

Vancomycin exhibits a pharmacodynamic profile which includes
time-dependent bactericidal activity with concentrations normally
achieved in-vivo, and moderate persistent effects. The pharmacodynamic
surrogate for efficacy is therefore AUC/MIC.
