What is everyones views on giving quinolones down a tube and being mixed with
enteral feedings such as Glucerna? Micromedix has a Drug Consult regarding
Cipro and enteral feedings but it doesn't commit one way or the other and
seems to recommend a cautious stance of holding the tube feeding for 4 hours
before until 2 hours after a dose. This would be 12 hours a day for bid
dosing. This seems to be a real problem as we try to push for IV to PO
quinolones in alot of our ICU patients but alot of them get tube feedings.
Can one increase the dose or give normal doses to patients with compromised
renal function to possibly compensate for any decreased absorption?

You could monitor the serum levels (or do a small study to look
at serum levels). Our department provides this service (Cipro
assay)
