Dear subscribers,
We have performed a pk study of vancomycine (at J1 and at J3) in ICU
patients (n = 20) following i.v. infusion (1hr) every 8 hr.
In comparison to J1, we have observed an increase in Vss (+54 %) and a
decrease in CL (-34 %) without apparent change in CLcr (estimated from
creatinine plasma levels by using Cockcroft and Gault equation).
What could explain these pharmacokinetic changes ?

Please define J1 and J3.
This may reflect the PK model you used to analyze your data, especially if
you are using a one-compartment model. The one-compartment analysis may not
reflect beta, and additional accumulation with subsequent doses will be
greater than that predicted by Ke.
Apparent Vd will also increase with multiple doses for a drug exhibiting two
compartment kinetics, as Vdss more closely approximates Vdbeta.
